How to order the service?

For legal entities:
- Form 4 – we will place the stamp and signature – DO NOT FILL IN, without date(template for download);
- Power of attorney – 2 pcs. (template for download).
- Original certificate from Statistics (a new one will be issued in exchange and the old one with a note).

For individuals:
- Form 11 – we will place the stamp and signature – DO NOT FILL IN, without date(template for download);
- Power of attorney – 2 pcs. (template for download).
- Copy of the Certificate of registration or Extract.

KVED
All newly established legal entities and entrepreneurs encounter KVED; when registering they must choose up to six types of activity and indicate them during state registration. The data on activities will be entered by the state registrar into the Unified Register and will also be indicated in your statistics certificate.
Keep in mind that legal entities and individual entrepreneurs must carry out their activities according to the types of activity they declared. Tax authorities will closely monitor compliance with this rule.
IMPORTANT!
It is important to know that for legal entities the declared types of activity must be explicitly mentioned in the company’s charter.
As of January 1, 2012, the new KVED 2012, corresponding to European standards, came into force, and therefore all legal entities and individual entrepreneurs are required to replace the activity codes indicated in the statistics certificates by the end of 2012.
You may be interested to know that the codes will now become four digits, and their names have in some cases changed significantly. Since automatic replacement of activity codes is not allowed, you must carry out changes to KVED 2012.
To change codes for legal entities you will need the following documents:
- previous statistics certificate, in exchange for which a new certificate will be issued (original);
- registration card Form 4;
- power of attorney 2 pcs.
To change codes for individual entrepreneurs you need the following documents:
- certificate of state registration or extract (copy);
- previous statistics certificate, in exchange for which a new certificate will be issued (if available);
- registration card Form 11;
- power of attorney 2 pcs.
You need to change the economic activity codes primarily for the following reasons:
- so that your founding documents comply with the current legislation of Ukraine;
- to provide new data to the tax office, since it is the tax authorities that will control the correctness of the activity code designation, as your taxation method will depend on this according to the requirements of the Tax Code of Ukraine;
- for further submission of data to the authorities that issued your enterprise licenses or permits to carry out certain types of activities.
Of course, changing activity codes does not necessarily depend on the introduction of the new KVED 2012; you may simply wish to change your company’s business profile. In that case, the set of documents required will be the same.
Given that in 2012 the change of activities causes some excitement, the queues in the corridors of state registrars and statistical offices are considerable.
