New forms of reports that must be submitted to the State Employment Service were adopted by orders of the Ministry of Social Policy No. 315 , 316 , 317 dated 31.05.2013 and came into force on July 12, 2013.

No. 1 – PA «Information on the number of citizens employed by business entities providing mediation services in employment».
This report must be submitted by individuals and legal entities – entrepreneurs whose activity consists of providing mediation services in the field of employment and job search, i.e., looking for personnel or hiring employees to perform work for other Ukrainian employers.
No. 3-PN «Information on labor demand (vacancies)».
If previously this report was required to be submitted only by legal entities, it now becomes mandatory also for individuals – entrepreneurs who plan to formalize employment relationships with hired workers. This form should contain the characteristics of the vacancy and the requirements imposed on applicants.
No. 4-PN «Information on planned mass dismissal of employees due to changes in the organization of production and labor».
From now on, this report must be submitted by all enterprises that plan mass dismissal of employees due to transformations in the company, but no later than 2 months before the date of dismissal.
In addition, the order of the Ministry of Labor and Social Policy of Ukraine that approved PN No.3 and No.4 in the current form has been revoked.
